Orchiopexy – testicular fixation

Orchiopexy is a surgical procedure in which a misplaced undescended testicle is moved to the correct position in the scrotum. The procedure should not be left too late – ideally it should be performed in the first 18 months of life. Having the testicle in the right position in the scrotum is essential for its proper development and function.

Causes

It is most often a congenital disorder in which the testicle does not descend correctly before birth. It either fails to descend into the scrotum (testicular retention) or ends up in another location (ectopia). A less frequently encountered problem is testicular ascension, where testicles originally located in the scrotum are relocated outside it. This can be caused by a congenital disposition or scarring of the inguinal canal.

Symptoms

The testicle is not located in the scrotum. It may be palpable outside the scrotum, for example on the groin or the inner surface of the thigh or it may be inside the abdomen, in which case it is impalpable. It is a common developmental defect affecting around 3% of all newborn boys.

Surgery

The operation is performed on children under general anaesthesia in a supine position and takes 50-60 minutes. If the patient has a palpable undescended testicle, the testicle is dislodged from the surrounding tissues through an incision in the groin and then fixed in a subcutaneous pocket in the scrotum (orchiopexy). In the case of an impalpable testicle, diagnostic laparoscopy must be performed first as this is the only procedure that can determine for sure whether the testicle is present in the abdominal cavity. The orchiopexy part of the surgery takes around 50 minutes and the diagnostic laparoscopy may take shorter or longer depending on the specific case.
